We’ve helped hundreds of people through the immigration exam process—here are answers to the questions we hear most often
No problem! We’ll review any records you have and help determine what’s missing. In many cases, we can order a blood test to check for immunity or help you get the vaccines you need.

We offer clear, upfront pricing—no surprise fees. Your total depends on what labs or vaccines you need, and we keep costs low, especially for uninsured patients.

This exam isn’t about “passing or failing.” It’s about documenting your health status for USCIS.
Health conditions are grouped into two categories:
Class A conditions require treatment or waiting before your application can move forward. These include things like untreated syphilis, active tuberculosis, or current drug addiction.
Class B conditions are very common—like diabetes, high blood pressure, or a past surgery. These don’t stop your application. I just document them accurately.
We’ll go over everything clearly and review your paperwork together to make sure it’s complete. The most important thing is to be honest about your health—USCIS is more concerned about incomplete or misleading information than about most medical conditions.

Yes. We are a bilingual, multicultural office that welcomes patients from all backgrounds. Se habla español. We’ve helped over a thousand immigrants through this process with kindness and professionalism. All of us have friends or family who are immigrants. This service is personal as well as professional.

Usually not. We do most required labs and vaccines in our office. If you need to go somewhere else, we will come up with a plan before you leave the office. Our goal is to make this as simple as possible.

After your appointment, most forms are ready within 3–7 business days. We’ll seal the I-693 form as required by USCIS and let you know when it’s ready.

"I had one of the best experiences with Dr. Cynthia. She was so nice, kind and helpful the whole time. I needed Medical Exam for USCIS and I was looking for affordable options around Cincinnati and North KY. I called a bunch of clinics and hospitals and nobody was nearly nice and helpful as Dr. Cynthia's clinic. I highly recommend!"

"Excelente servicio y asesoría de parte de la Doctora Villacis y su equipo.
Aparte de muy profesional y cordial, para los latinos que estamos en los primeros pasos del idioma inglés, con ella no se necesita un intérprete.
Ya que habla muy bien el español y por ende existe una excelente comunicación a la hora de la consulta.

Dr. Cynthia Villacis, MD
Bilingual Family Physician, Civil Surgeon and Life Coach
I’m Dr. Cynthia Villacis, a family physician with over 20 years of experience—and someone who has walked many of the same health journeys as my patients. I’ve faced challenges with weight, blood sugar, stress, and sleep, and I understand how frustrating it can feel to do “all the right things” and still not get answers.
My approach to care is shaped by both personal experience and a lifelong exposure to service and human-centered values. I was raised in a Unitarian Universalist tradition and grew up in a family rooted in advocacy—both of my grandfathers were American Baptist ministers, and my mother worked in social services. From an early age, I was exposed to the women’s movement, domestic violence shelters, and community programs for adults living with serious mental illness. These experiences taught me that health is inseparable from dignity, safety, education, and support.
That foundation led me to spend more than 12 years working in community health centers and ultimately to create Health Connections DPC—a practice designed to offer unhurried, judgment-free, whole-person care. I take time to listen, identify root causes, and help patients build realistic plans that fit their lives. I proudly serve a diverse and inclusive community, including immigrants, LGBTQ+ individuals, and Spanish-speaking families, with respect and compassion.
I completed my Family Medicine residency in 2002 and have spent over two decades helping patients manage chronic conditions such as diabetes, hypertension, and obesity through a root-cause approach grounded in lifestyle medicine. I am board certified in Family Medicine, Lifestyle Medicine, and Addiction Medicine, with special interests in obesity management and metabolic health, as well as mental health, women’s health, and immigrant health. I believe health begins with understanding, and that education is the foundation for lasting change.
After years in a busy community health center, I recognized that 10–15 minute visits were not enough—especially for patients navigating stress, trauma, weight changes, or hormonal transitions. In 2017, I opened my own practice to offer longer visits, easier access, and relationship-based care that allows real healing to happen.
My practice is fully bilingual in English and Spanish. I learned Spanish while living in Ecuador as an exchange student in 1988—where I also met my future husband—and I remain deeply committed to caring for immigrant families. I am also a designated civil surgeon, providing USCIS I-693 immigration medical exams in a compassionate, low-stress environment.
Outside the office, I’m a proud mom of two grown children, wife of over 30 years, and an outdoor enthusiast. I enjoy kayaking, yoga, hiking, scuba diving, and exploring new places—and I believe joyful movement and adventure are possible at any size. My own journey with metabolic health and self-compassion continues to inform the care I offer my patients.
If you’re looking for a doctor who sees the full picture—body, mind, and lifestyle—and who truly takes the time to listen, I’d love to welcome you to Health Connections DPC.

👩💼 Meet Gisselle
Bilingual Medical Assistant & Office Manager
I first joined Health Connections DPC in 2023 and returned in the fall of 2024 after completing my Associate’s Degree in Business. It’s an honor to now serve as both a medical assistant and the office manager, supporting our patients and keeping things running smoothly behind the scenes.
Originally from Puerto Rico, I moved to Kentucky in 2017 and have been dedicated ever since to helping people from all backgrounds feel cared for and understood. As a bilingual team member, I take pride in bridging language and cultural gaps — ensuring that both English and Spanish-speaking patients receive the clear, compassionate care they deserve.
I love learning and am always looking for ways to grow in my role and serve our community even better. Outside of work, I enjoy reading, knitting, and spending quality time with my family — the people who keep me grounded and inspired.
🕊️ Kind, professional care. Clear answers. We’re here to guide you through it—start to finish.
